Cosmetic Plastic Surgery Institute does not collect identifiable or personal information of its internet users unless the user voluntarily provides such information. In addition, we do not disclose or share your photograph or identifiable information without your consent unless required by law or court order.
All personal information, photographs, and patient concerns will be stored on a secure in house database per HIPPA guidelines and only be accessed by our in house board certified physicians at Cosmetic Plastic Surgery Institute in order to provide you a complimentary online consultation.
The online consultation provided by our staff physician is meant to serve as a screening tool for our users and should be considered tentative in nature. Appropriate in-person consultation by our in-house physicians is necessary to provide definitive diagnoses regarding any skin concerns. An in-person consultation may be obtained which will include obtaining a patient history and a hands on physical examination in order to make medical recommendations.
Terms of this agreement will authorize a Cosmetic Plastic Surgery Institute staff physician to review your personal information, patient concern, and/or provided photographic image of your lesion. Review of these items will allow our physician to provide you a tentative and provisional diagnosis and recommendation plan.
Cosmetic Plastic Surgery Institute reserves the right to modify its Terms and Agreement policy as deemed necessary. It is your responsibility to review this Terms and Agreement policy each and every time future complimentary on-line consultations are requested.
